Every decision that changes the playbook lives here, numbered, with rejected options left visible. The rejected options are the evidence of judgment. A decision without a recorded alternative was not a decision.

Statuses: Drafted → Accepted → (Superseded by ADR-NNN if revisited). Closed decisions are never edited; they are superseded.
